Admission Criteria
Acceptance/Admission of client into this service shall require the following:
- Recent medical history including hospitalizations
- Significant medical conditions affecting functioning
- Other active and significant chronic or acute medical diagnosis
- Known allergies to drugs and foods
- Medical confirmation that the individual is free from reportable active communicable diseases
- Current and other needed medications
- Current and other needed treatments and services for medical conditions and related problems
- Current nutritional status
- Fall risk and Braden Scale Assessments
- Medically necessary limitations or precautions
- Post-admission tests and follow-up appointments
Client’s functional ability and psychosocial circumstances
- Level of functioning in Activities of Daily Living (ADL)
- Level of support and intervention needed, including any special equipment and supplies required to compensate for the individual’s deficit in ADL
- Current physical or psychosocial symptoms requiring monitoring, support or other intervention by the caregiver
- Capacity for making personal and healthcare-related decisions
- Presence of disruptive behavior or behavior which may present a risk to the health and safety of the client or others
- Social factors such as significant problems with family circumstances and personal relationships, ability to participate in group and structured activities, spiritual status and needs.
